1.Vaders first breath. Brilliant. A really good scene, marking the birth of one of cinemas greatest iconic villains ever. Then he goes Frankenstein "Nooooo!!" and the scene immediatly goes down the toilet. Goddammit George!!
2. How unbelievably stupid is the script anyway? Especially when it comes to Anakins turn. This is IMHO the biggest flaw, not only from a logical perspective but also an emotional one. It is the entire point for making the PT.
3. Padmé feels like dying. "There is nothing physically wrong with her..."

4. "I have the higher ground." What?? We're talking about Jedi here. They jump. Really high. Again, Lucas feels the urge to make idiotic explanations where none are needed.
5. Where's Wedge? Red Two? Porkins for f**ks sake!? Jedi, lotsa droids and clones but no ordinary people to care for. The action scenes are all sensory overload, not the drama enhancing excitement of the OT. Yes, there are Jedi and Jedi children being killed but it doesn't provoke the emotions and outrage it should. It's like Lucas is afraid to portray Anakin as the murdering bastard psycho he is.
6. Wookies are cool, I like them. Did they add anything to this movie? No.
7. Palpatine said "midichlorians" and my ears went numb.
8. Almost everything attempting to tie RotS to ANH feels contrived. So C-3PO and Artoo spends 18 years on the Blockade Runner with Captain Antilles? Lame!

There's lots more, but above all, what I am disappointed in is the wasted potential. I can sense an amazing, powerful movie somewhere under all that crap. (I couldn't when it comes to the previous PT installments). And on a few occasions it does shine through. I'm a sucker for binary sunsets, and in that last scene I saw a glimpse of Star Wars.
